Black Infant Health (BIH)

Services

A group-based, social empowerment approach coupled with case management activities. Ten-week prenatal and postpartum  group sessions. One-on-one support is available for participants not able to attend group sessions. Services provided by BIH in both Area 1 and Area 2 are available to all eligible residents of Alameda County. 


Program Eligibility

  • Black/African American women age 16 years and over, pregnant up to 6 months postpartum living in Alameda County. 
  • Cut off age 45 years old.

Services 

Building Bridges provides nursing case management services for at risk pregnant and post-partum women and their families. 

Public health nurses conduct physical and psychosocial assessment, education, and care planning. 


Program Eligibility 

  • Building Bridges Pregnant and post-partum women and infants ages 0 to 12 months old.
  • Families experiencing a SIDS death.

Hayward Promise Neighborhood  (HPN)

Services

   A public health nurse provides multicultural/multilingual case management, outreach, and care coordination, focusing on intensive prenatal and postpartum support for high-risk, low-income families in the Hayward Promise Neighborhood. 


Program Eligibility

  •  Pregnant or post-partum mothers, families with a child 0 to 8 years old living in targeted Downtown Hayward area and schools.

Hayward Promise Neighborhood

DREAMS: Desired Reproductive Health Access for Maternal Services

 Client-centered case management and group support services to pregnant and interconception individuals and couples with a history of adverse pregnancy outcomes.  


Program Eligibility  

 

  •  Prior fetal loss, neonatal death, or multiple miscarriages, including those expecting fetal demise. 
  • Medi-Cal eligible residents of Alameda County.
  • African American/Black residents of Alameda County, regardless of income level.

 Virtual and in-person perinatal groups support Black women's well-being, healthy pregnancies, child development, and trauma recovery. Programs include EmbraceBIRTH childbirth education and BElovedBIRTH Black Centering prenatal care (with Alameda Health System).

 Promotes responsible fatherhood and healthy relationships through parenting education, skill building, male-centered case management, and support programs including Boot Camp for New Dads, Support Groups, and Home Visitation/Case Management.

 A home visiting program by trained social workers promotes maternal and child health, enhances parent-child interactions, and supports child development and family protective factors. Visits start prenatally or post-birth and end at age three.

  • Non-CalWORKs: Prenatal referrals.
  • CalWORKs:
    • Prenatal through 6 weeks post-partum.

 Nurse Family Partnership Services

Evidence-based, intensive, comprehensive home visiting program by trained public health registered nurses.

Designed to improve prenatal health and outcomes, child health and development, family economic self-sufficiency, maternal life course development.

Visits begin early in pregnancy and conclude when the child turns two years old.

  • First time moms before 28 weeks gestation.
  • Alameda County resident.

Blue Skies Mental Wellness Program (BSMW) Services

 Blue Skies provides community-based mental health support to prenatal mothers and children under two enrolled in Starting Out Strong programs. Services include therapy, case management, and family support, specializing in postpartum depression, anxiety, trauma, and parent-infant bonding.

  •  Alameda County residents with babies under two years of age. 
  • Pregnant mothers or parents enrolled in Starting Out Strong programs.
  •  Free prenatal groups  open to all pregnant/postpartum Alameda County residents.

Doula Services Medi-Cal Benefit

Doula Services Medi-Cal Benefit 

Doula Care encompasses the following: the creation of a Birth Plan, Health Navigation, facilitating connections to community resources, and Lactation Support.

The services offered include an initial prenatal visit, eight prenatal and postpartum care visits, support during labor, delivery, abortion, or miscarriage, extended postpartum visits lasting two to three hours, and virtual or in-person doula services.

  • Currently pregnant or were pregnant within the last year.
  • Must have current coverage under Medi-Cal Managed Care.

California Abundant Birth Project

California Abundant Birth Project

California Abundant Birth Project

California Abundant Birth Project

 California Abundant Birth Project will offer monthly unconditional income supplements to 950 pregnant individuals at high risk of perinatal health disparities, starting in the 1st or 2nd trimester for 12-18 months from January 2024. Participants in Alameda County will receive $967 monthly. 


Program Eligibility 

  • Live in Alameda County.
  • Be 8-27 weeks pregnant at the time of Abundance Drawing. 
  • Have household income under $128,017
  • Identify with one or more of the following risk factors for preterm birth:
    • Are Black or African American 
    • Have had a previous preterm birth (live before 37 weeks)
    • Have preexisting hypertension (before this pregnancy)
    • Have preexisting diabetes (before this pregnancy)
    • Have sickle cell anemia (SCA)
  • Not be currently participating in another guaranteed income program.
